Thank you for taking the time to browse through Living Without Boundaries website. We have provided for you here a link to our Occupational Therapy Intake form and consent form.
Please download/print for completion and bring with you to your first visit. (Format is best accessed via a computer not a tablet or phone)
By completing these forms, we will save time and will get the most out of your session.
All you need to bring is yourself! You will receive a thorough assessment and evaluation in regards to your concerns and a plan of therapy will be discussed prior to treatment. If you have any relevant documentation, ie: GP letter, scan reports or information from other Health professionals involved. Please bring along with you.
Occupational Therapy Initial consult items
Your first appointment with an Occupational Therapist requires a lot of information gathering so we can get to know you and your child/ family better. Please bring the following if you have available to you.
– Referral Letters
– Health Practitioner reports
– Funding papers
– Scans (if necessary)
– Independent Learning Plans (ILP) from the school
– OT Intake Documentation ( or you may scan and email back prior to session)
